If you are a current or former Creative Harmony Clinical Services client and are interested in requesting a copy of your treatment records, please complete the below form. Creative Harmony Clinical Services, LLC is required to keep your files for no more than seven years since your discharge from services. Please note that all records request must meet HIPAA requirements , therefore, there must be an appropriate Release of Information form on file. Once your request is received, you will be contacted by phone by one of our team members to finalize the request process. You may be required to visit the office during business hours to fill out appropriate forms. Please note, if your request is received on the weekend it will be processed during the next business day.
